The call ends with a proposed treatment plan.<\/li>\n<\/ul>\n<h4 style=\"color: #532f64;\">Pricing Plan<\/h4>\n<ul>\n<li>We contact your insurance provider to produce a pricing plan and to determine the extent of your benefits.<\/li>\n<li>You are given the treatment plan and payment breakdown of Dr. Cook\u2019s surgical treatment fees* within 2 working days of your record review.<\/li>\n<\/ul>\n<p><em>* Dr. Cook\u2019s surgical treatment fees include the pre-op phone and in-office consultations, surgery, and all post-op phone and in-office consultations relating to surgery up until 1 year following your surgery date.<\/em>[\/vc_column_text][\/vc_tta_section][vc_tta_section title=&#8221;Phase 2: Pre-op planning and consultation&#8221; tab_id=&#8221;1556059035387-a991dc3a-accd&#8221;][vc_column_text]<\/p>\n<h4 style=\"color: #532f64;\">Scheduling<\/h4>\n<p>Pre-op, surgery and post-op appointments are scheduled.
